Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families in their homes and communities. You will develop treatment plans, deliver therapeutic interventions, and connect families to needed services while maintaining thorough documentation and regulatory compliance.

The role involves active referral recruitment, 24/7 on-call support as required, and collaboration with the clinical supervisor to tailor care.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care with the treatment team.
• Provide individual, group, and family therapeutic interventions as outlined in the plan.
• Attend weekly Treatment Team meetings and recruit new referrals.
• Maintain accurate documentation of client interactions and progress notes.
