Job summary

Health Connect America, Inc. is seeking a Family Development Specialist to support clients and families by creating and implementing treatment plans, delivering therapeutic interventions, and linking clients to needed services in home and community settings.

You will work within a collaborative treatment team, maintain thorough documentation, and actively recruit referrals to sustain caseloads, with 24/7 on-call support as needed and strict adherence to COA standards.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families, including internships or volunteer work.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building as outlined in the Treatment Plan.
• Attend and participate in weekly Treatment Team meetings.
• Link clients and families to services and resources identified in the treatment plan.
• Serve as a liaison with other professionals and community resources to meet client needs.
• Maintain accurate documentation of client interactions, treatment plans, and progress notes in compliance with organizational policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development to stay current with best practices.
