Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by creating and implementing treatment plans and delivering evidence-informed therapeutic interventions in community settings. This role involves coordination with the treatment team, thorough documentation, and active recruitment of new referrals.

Qualifications

• Bachelor’s degree in human services such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or related field.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions as outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the treatment plan.
• Liaise with other professionals, agencies, and community resources to meet client needs.
