Job summary

Health Connect America seeks a dedicated Family Development Specialist to develop and implement treatment plans and provide therapy to children and families in community settings. You will coordinate care with the treatment team and supervisor, delivering individual, group, and family interventions with thorough documentation.

You will link clients to services, recruit referrals, and ensure regulatory compliance while working in homes and communities.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families (internships/volunteer work included).
• This is a community-based position working within clients' homes.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building as outlined in the Treatment Plan.
• Attend and participate in weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the treatment plan.
• Serve as liaison with other professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of client interactions, treatment plans, and progress notes per policy and standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development.
