Health Connect America is seeking a Family Development Specialist to support clients and families through treatment planning, therapeutic interventions, and service linkage in a community-based setting, including in clients’ homes.

The role emphasizes collaboration with the treatment team, thorough documentation, and active referral recruitment, while ensuring regulatory compliance and ongoing professional development.

Qualifications

• Bachelor's degree in a human services field required.
• Minimum of two years experience with children and/or families.
• Internships/volunteer work acceptable to meet experience requirement.

Responsabilités

• Develop individualized treatment plans with input from the treatment team.
• Provide individual, group, and family therapy as outlined in plans.
• Attend weekly Treatment Team meetings and participate actively.
• Link clients to services and resources identified in plans.
• Maintain timely documentation of client interactions and progress.
• Recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required.
• Ensure compliance with state regulatory bodies and COA standards.
• Pursue ongoing training to stay current with best practices.
