Job summary

Health Connect America seeks a Family Development Specialist to support clients and families in home and community settings. You will develop treatment plans, provide therapeutic interventions, and connect families with needed services, while ensuring compliance with regulatory standards.

The role emphasizes effective communication, documentation, and active referral recruitment, with 24/7 on-call support as required by the program, and a commitment to COA standards and client-centered care.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience with children and/or families.
• Background checks and regulatory compliance required.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team.
• Provide individual, group, and family therapeutic interventions as outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources identified in the treatment plan.
• Serve as liaison with other professionals and community resources to meet client needs.
• Maintain accurate documentation of all client interactions, plans, and progress notes per policy.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
