Job summary

Health Connect America is hiring a Family Development Specialist to support clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to necessary services. This is a community-based role conducted in clients' homes and communities.

You will work with a treatment team, maintain documentation, and actively recruit referrals to sustain a full caseload, while complying with COA standards and state regulations.

Qualifications

• Bachelor’s degree in a human services field is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team.
• Provide individual, group, and family therapeutic interventions, and skill-building as outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources identified in the treatment plan.
• Serve as a liaison with other professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of all client interactions, treatment plans, and progress notes in compliance with policy and standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development.

Education

Bachelor’s degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field
