Carewell is seeking a dedicated Care Concierge in the Charlotte, NC area to support enrollment, welcome visits, and ongoing care navigation for older adults. You will lead outreach, obtain verbal consents, and coordinate provider appointments as part of a cross-trained pod.

This early-stage program evolves with your feedback and collaboration to close care gaps and connect members to community resources. The role requires CMA/RMA credentials and 1+ years of member-facing healthcare experience,

Qualifications

• CMA/RMA credential required (nationally recognized).
• 1+ years of member-facing healthcare experience.
• Experience with Medicare-enrolled, dual-eligible, or older adult populations.
• Proven ability to build trust with older adults and communicate effectively.

Responsibilities

• Enroll prospective program participants by outreach and consent.
• Conduct welcome visits with new members and set expectations for ongoing coordination.
• Coordinate care navigation, close care gaps, and connect members to resources.

Education

Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) credential
