ArchWell Health in Greensboro, NC, is seeking a Care Navigator to welcome members and their families, guide them from check-in through discharge, and ensure care needs are met with a positive, member-centered experience.

The role requires excellent customer service, ability to work with diverse populations, and familiarity with EMR systems; associates degree preferred, plus experience in clinical settings. Fluency in Spanish is a plus and a team-focused attitude is essential.

Qualifications

• Associates degree preferred, or equivalent experience.
• Minimum of one year of work experience in a clinical setting.
• Experience working with EMR systems is a plus.

Responsabilités

• Welcome members upon arrival and guide them through check-in and the care process.
• Update member information in the EMR system.
• Schedule appointments and coordinate referrals for a multidisciplinary care team.
• Assist with paperwork via electronic kiosks and documentation uploads.
• Answer questions from members and families and manage center communications.
• Support center events and coordinate with staff as needed.
