Job summary

Carewell is hiring a Remote Care Concierge to support older adults with high-risk chronic conditions through proactive care coordination, education, and advocacy. The role focuses on maintaining trust, navigating complex healthcare systems, and reducing barriers to care in a fully remote setting.

You will manage a caseload, collaborate with clinical teams, and use multiple outreach strategies to ensure patients understand options and receive needed services.

Qualifications

• Must have CMA or RMA credential from a nationally recognized certifying organization.
• 1+ year of remote care coordination experience (CCM, RPM, care navigation).
• Experience with Medicare-enrolled or dual-eligible populations.
• Ability to build trust and communicate effectively with older adults with chronic conditions.
• Strong understanding of care coordination principles and healthcare systems.
• Proficient with EHRs, care management platforms, CRMs, and digital tools.
• Self-directed, metric-aware, able to manage own caseload.
• Must be located in one of the following states: FL, GA, NC, TN, or TX.

Responsibilities

• Serve as the primary contact for enrolled patients, building trust through compassionate outreach.
• Conduct regular check-ins to assess health status and social barriers.
• Maintain a caseload of approximately 75-150 patients, prioritizing acuity and gaps.
• Coordinate with family members and caregivers when appropriate.
• Navigate the healthcare system and coordinate appointments and plans.
• Liaise with PCPs, specialists, pharmacies, home health, and community resources.
• Escalate clinical concerns promptly to the supervising LVN.
• Document all patient interactions in real time with accuracy.
• Support medication adherence and insurance benefit exploration.
• Connect patients with durable medical equipment and transportation.
