Job summary

Carewell in the United States is expanding its Care Concierge program to guide members from enrollment through ongoing care navigation. You will work in a cross-trained team handling enrollment, welcome visits, and care coordination with daily adaptability to queue volume and evolving processes.

Ideal candidates are CMAs or RMAs with 1+ year of member-facing healthcare experience, comfortable with older adults, and proficient with EHRs and care platforms.

Qualifications

• CMA/RMA credential required; minimum 1+ year of member-facing healthcare experience.
• Experience with Medicare-enrolled/older adult populations preferred.
• Strong communication, coordination, and compliance mindset required.

Responsibilities

• Enroll and secure participation; conduct outbound outreach with structure.
• Welcome new members and set expectations for ongoing coordination.
• Coordinate care navigation: liaise with providers, pharmacies, and community resources.
• Document interactions in real-time; follow consent and compliance gates.
• Work across enrollment, welcome visits, and care navigation as needs shift.
