ArchWell Health is seeking a Care Navigator to deliver a welcoming experience for members and their families, guiding them from arrival to departure with a warm, service-oriented approach and a strong commitment to senior care.

The role focuses on compassionate member support, efficient check-in, and collaboration with a multidisciplinary team to meet care needs and ensure a smooth center visit.

Qualifications

• Associates degree preferred or equivalent experience.
• Minimum of one year of work experience in a clinical setting or similar.
• Experience working with EMR systems is a plus.

Responsibilities

• Welcome members upon arrival and guide them through check-in.
• Update member information in the EMR system.
• Schedule appointments and coordinate referrals for a multidisciplinary care team.
• Assist members with filling out paperwork through electronic kiosks as needed.
• Request medical records and upload documentation to EMR.
• Field questions from members and their families.
• Manage center phone line by taking detailed messages and conveying details.
• Assist with center events as needed.
