Alignment Health is seeking a Remote Regional Concierge Navigator to provide outreach and support to our members, ensuring access to care and benefits. You will connect our provider network with health plan operations and vendors, guiding members through onboarding, scheduling, and coordinated care while maintaining knowledge of market needs.

Qualifications

• Knowledge of ICD-10 and CPT codes.
• Typing 40+ words per minute.
• Ability to help members navigate access to care through Medicare Advantage or HMO, including referrals and authorizations.
• Effective written and oral communication skills; ability to establish and maintain a constructive relationship with diverse members, management, employees and vendors.

Responsabilités

• Provides in-market, specialized member support in respective market or region.
• Engages members to onboard, schedule, and coordinate care with PCPs and providers.
• Ensures access to PCP and specialists; coordinates care and authorizations.
• Educates members on gaps in care and assists with scheduling provider appointments.
• Acts as liaison throughout the member life cycle and supports wellness plan options.
• Keeps regular outreach to members via calls, letters, and reminders.
• Handles member eligibility, benefits, and provider information inquiries.
