Job summary

Carewell is seeking a dedicated Care Concierge in Fort Lauderdale to join our care navigation team. You will guide new members from enrollment through ongoing coordination, bridging enrollment, welcome visits, and care navigation to close gaps and connect to community resources.

You’ll conduct outreach, welcome visits, and coordination with providers, pharmacies, and social services, using plain language, empathy, and trauma-informed care to support older adults and their families.

Qualifications

• 1+ years of member-facing healthcare experience (e.g., clinical front office, enrollment, care coordination).
• Ability to build trust with older adults and communicate clearly.
• Familiarity with care coordination principles and navigating healthcare systems.
• Proficient with EHRs, CRMs, and digital communication tools.

Responsibilities

• Enroll prospective participants and secure consent for enrollment.
• Conduct member welcome visits and set expectations for coordination.
• Coordinate with primary care providers, pharmacies, and community resources to close care gaps.
• Maintain accurate real-time documentation and comply with gates and audits.
• Escalate care navigation concerns promptly and maintain a regional resource directory.
