Job summary

Imagine Pediatrics is a tech-enabled pediatric medical group delivering 24/7 virtual and in-home care across the US. The Patient Engagement Associate II collaborates with a pod to enroll children and caregivers, using outreach to families and providers to remove enrollment barriers.

The role emphasizes critical thinking, collaboration, and autonomy in a startup environment, with a compensation package including hourly pay and annual bonus, plus comprehensive benefits.

Qualifications

• 2+ years of healthcare experience in care coordination, community health, patient engagement, customer service, or sales.
• Proven success in a centralized contact center environment.
• Experience with resourcing within a patient or family facing role and navigating social service agencies.
• Strong aptitude for documentation; prior experience with a CRM platform is a plus.
• Excitement to be part of a tight-knit team with shared accountability for enrollment; excellent collaboration skills.
• Stellar active listening and communication skills; ability to tailor value proposition to various audiences and overcome complex objections.
• Strong ability to build trust and relationships with caregivers and other healthcare organizations.

Responsibilities

• Conduct thorough research on attributed patients to understand healthcare needs and barriers to enrollment.
• Perform telephonic outreach to caregivers to enroll children in care and ensure seamless onboarding.
• Contact other healthcare professionals involved with attributed patients (DME, home health, pharmacies, pediatricians).
• Leverage internal and external relationships to support ongoing engagement and care coordination.
• Handle inbound calls from caregivers, health plan service coordinators, and hospital discharge planners during enrollment.
• Develop knowledge of SSI program and outreach to eligible caregivers.
• Collaborate with Provider Engagement Specialists for on-site outreach to support enrollment.
