Elevance Health is seeking a LTSS Service Coordinator (Bilingual in Spanish/English) to coordinate long-term supports and services for a defined caseload. The role involves field-based assessments, care planning, and collaboration with health professionals to ensure integrated care plans and member-centered goals.

The position requires travel across multiple Florida counties, with hybrid work considerations and the possibility of occasional office attendance for meetings or training.

Qualifikationen

• BA/BS degree required or equivalent experience.
• Minimum 2 years of experience with a social work agency; state requirements may apply.
• LTSS, case management, social work or hospital discharge planning experience preferred.
• LTSS Service Coordinator role requires cross-functional collaboration and planning.

Aufgaben

• Conduct face-to-face program assessments using predefined questions and motivational interviewing.
• Coordinate and manage waiver services (LTSS/IDD), BH or PH needs with interdisciplinary team.
• Document short- and long-term service goals in the Person Centered Planning process.
• Identify members who may benefit from alternative levels of service or other waivers.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state/county agencies and participate in audits.
