Job summary

Elevance Health is seeking a field-based LTSS Service Coordinator to support a defined caseload and collaborate with a multidisciplinary team to develop Person Centered Plans. The role emphasizes in-person assessments, care coordination, and engagement with members, caregivers, and providers.

The position requires a BA/BS degree and at least 2 years in a social work setting; Spanish bilingual ability is strongly preferred, with prior LTSS or discharge planning experience advantageous.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience which would provide an equivalent background.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.
• Bilingual in English/Spanish strongly preferred.
• LTSS, case management, social work or hospital discharge planning experience preferred.

Responsibilities

• Conduct face-to-face program assessments for identification and case management.
• Coordinate and manage an individual’s waiver and health needs with the interdisciplinary team.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective utilization of LTSS.
• Document short and long-term service goals with the member and care team.
• Identify Members who could benefit from alternative levels of service or other waiver programs.
• Mentor or preceptor roles for new staff and contribute to process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state and county agencies as required.
