Job summary

Qualifications

• BA/BS degree required; or equivalent combination of education and experience.

Responsibilities

• Perform face-to-face program assessments using predefined questions and motivational interviewing techniques.
• Coordinate and manage an individual’s waiver and health needs with the inter-disciplinary team.
• Document short and long-term service goals with the care team.
• Identify members who could benefit from alternative services or other waiver programs.
• Submit utilization/authorization requests with proper documentation.
