Job summary
Heartland Hospice is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays. You will be the primary liaison between patients, families, physicians, and the hospice team, delivering direct patient care and symptom management in home settings.

Qualifications
- Current RN license in the state of employment.
- At least 1 year of nursing experience.
- Compassionate, calm, and dependable in urgent care settings.
- Excellent clinical judgment and ability to support emotionally intense situations.
- Comfortable providing care in patients' homes or other locations.
- Valid driver’s license, automobile insurance, and reliable transportation.
- Current CPR certification.
Responsibilities
- Respond to patient needs after normal business hours.
- Serve as liaison between patients, families, physicians, and the hospice team during your shift.
- Document visits and communications accurately in the EMR.
- Provide direct patient care and symptom management as part of the care plan.
- Collaborate with the interdisciplinary group to ensure continuity of care.
