Job summary

Heartland Hospice is seeking an After Hours/On-Call Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays in patients' homes or facilities. You will collaborate with an interdisciplinary team, document care, manage symptoms, and guide families through end-of-life care with compassion and clinical excellence.

Qualifications

• Current RN license in the state of employment or eligibility to obtain
• 1+ years of nursing experience
• Comfortable providing care in patients’ homes, facilities, or wherever they reside
• Valid driver’s license and reliable transportation
• Current CPR certification

Responsibilities

• Provide direct patient care and support after normal business hours
• Serve as the primary liaison between patients, families, physicians, and the hospice team during shifts
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care
• Document clinical visits and communications accurately
• Participate in quality assessment and performance improvement programs
