Job summary

Heartland Hospice seeks an After Hours RN to provide compassionate, patient-focused care in homes and other settings during evenings, nights, weekends, and holidays. You will collaborate with the Interdisciplinary Group, document care in EMR, and ensure continuity of care when the daytime team is unavailable.

Ideal candidates hold an RN license in the required state, with 1+ year of nursing experience and CPR certification.

Qualifications

• Current RN license in the state of employment or eligibility to obtain.
• Minimum 1 year of nursing experience.
• Hospice/home health experience preferred but not required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care and support after normal business hours.
• Serve as primary liaison between patients, families, physicians, and hospice team.
• Respond promptly to urgent calls and symptom management needs.
• Document visits and communications accurately in EMR.
• Collaborate with interdisciplinary team to ensure continuity of care.
• Maintain Plan of Care and participate in QA/QI programs.
• Maintain infection control and safety practices.
