Heartland Hospice is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care in the home setting. You’ll act as the primary liaison among patients, families, physicians, and the hospice team, ensuring timely visits and clear communication.

You will evaluate patient needs, guide clinical decisions, and document all interactions.

Qualifications

• Current RN license in the state of requested employment (or eligibility to obtain).
• 1+ years of nursing experience.
• CPR certification preferred.
• Valid driver’s license and reliable transportation.

Responsabilités

• Respond to patient needs after hours, including evenings, nights, weekends, and holidays.
• Serve as liaison between patients, families, physicians, and the hospice team during shifts.
• Document visits and communications accurately in the EMR.
• Evaluate patient and family needs to guide care plans and symptom management.
• Collaborate with interdisciplinary team to provide holistic services and bereavement support.
