Job summary

Heartland Hospice seeks an After Hours/On-Call RN to deliver compassionate care in patients' homes during evenings, nights, weekends, and holidays. You will respond to urgent needs, coordinate with families, physicians, and the daytime team while maintaining documentation and adherence to the Plan of Care.

You’ll work independently yet stay connected with an interdisciplinary group, ensuring continuity of care, symptom management, and high-quality hospice services after hours.

Qualifications

• Current RN license in the state of requested employment (or eligibility to obtain).
• 1+ years of nursing experience.
• Hospice heart: compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and the ability to provide support in emotionally intense settings.
• Comfortable providing care in patients' homes, facilities, or wherever they reside.
• Valid driver’s license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Provide direct patient care after normal business hours and respond to urgent needs.
• Serve as liaison between patients, families, physicians and hospice team during shifts.
• Accurately document visits and communications.
• Collaborate with Interdisciplinary Group to implement Plan of Care.
• Support pain and symptom management and educate patients and families.
• Maintain high-quality care and compliance with policies and regulatory requirements.
