Job summary

Heartland Hospice is seeking an After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays. You will provide direct care, act as the primary liaison among patients, families, physicians, and the hospice team, and document care in the EMR with a focus on compassionate symptom management.

You will collaborate with LPNs/LVNs, CNAs, and other staff to evaluate needs and adjust the Plan of Care, while supporting quality improvement initiatives

Qualifications

• Active RN license in the employed state or eligibility
• 1+ years of nursing experience
• Hospice heart: compassionate, calm, dependable
• Strong clinical judgment for emotionally intense settings
• Comfort providing care in patients' homes or residences
• Valid driver’s license and reliable transportation
• Current CPR certification

Responsibilities

• Provide direct patient care during after-hours, evenings, weekends and holidays
• Serve as liaison between patients, families, physicians and hospice team
• Document visits and communications accurately in EMR
• Assess patient and family needs and update the plan of care
• Coordinate with Interdisciplinary Group for comprehensive care
• Support quality assessment and improvement programs
• Maintain continuity of care and coordination with daytime team
