Job summary

Heartland Hospice is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays. You will serve as the primary liaison among patients, families, physicians, and the hospice team, delivering direct patient care and coordinating with the daytime care team.

The role requires an active RN license in the state of employment, 1+ year nursing experience, and strong clinical judgment.

Qualifications

• Current RN license in the state of employment or eligibility to obtain.
• 1+ years of nursing experience.
• Hospice heart: compassionate, calm, dependable in urgent care situations.
• Excellent clinical judgment and ability to support in emotionally intense settings.
• Comfortable providing care in patients’ homes or care facilities.

Responsibilities

• Respond to patient needs after normal business hours and serve as liaison among patients, families, physicians, and hospice team.
• Provide direct patient care and support during evenings, nights, weekends, and holidays.
• Document visits and communications accurately.
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care.
• Collaborate with Interdisciplinary Group to provide holistic care including bereavement support.
