Job summary

Heartland Hospice is seeking an After Hours RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care in patients' homes or other settings.

You will report to the Administrator or Patient Care Manager, collaborate with an interdisciplinary team, document visits, and participate in quality improvement efforts while maintaining continuity of care.

Qualifications

• Current RN license in the state of employment.
• 1+ year nursing experience; hospice/home health preferred.
• Compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and communication.
• Able to provide care in patients' homes or other settings.
• CPR certification and valid driver’s license.

Responsibilities

• Respond to patient needs during evenings, nights, weekends and holidays.
• Provide direct patient care and symptom management.
• Coordinate with the Interdisciplinary Group and daytime team.
• Document visits and communications accurately.
• Participate in quality assessment and performance improvement programs.
• Promote continuity of care and safety.
