Job summary

Heartland Hospice is seeking an After Hours Registered Nurse to deliver compassionate care during evenings, nights, weekends, and holidays, often in patients' homes. This role requires strong clinical judgment, excellent communication, and the ability to work independently while coordinating with the daytime team.

You will report to the Administrator or Patient Care Manager Senior, document visits in EMR, and participate in quality improvement initiatives to ensure the highest standard of care

Qualifications

• Current RN license in the state of employment or eligibility to obtain
• 1+ years of nursing experience
• A hospice heart: compassionate, calm, and dependable in urgent care situations
• Excellent clinical judgment and the ability to provide support in emotionally intense settings
• Comfortable providing care in patients’ homes, facilities, or wherever they reside
• Valid driver’s license, automobile insurance, and reliable transportation
• Current CPR certification

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays
• Provide direct patient care and support after normal business hours
• Serve as primary liaison between patients, families, physicians, and the hospice team during shifts
• Document clinical visits and communications
• Participate in quality assessment and performance improvement programs
• Work independently while coordinating with daytime care team
• Monitor and manage pain and symptom control in the dying process
