Job summary

Heartland Hospice is seeking an After Hours RN to respond to patient needs during evenings, nights, weekends, and holidays. You will work independently, coordinate with the daytime team, and serve as the primary liaison among patients, families, and physicians to ensure timely, compassionate care at home.

You will assess patients, document visits, participate in quality improvement, maintain EMR notes, and provide bereavement support as part of an interdisciplinary team.

Qualifications

• Current RN license in the state of requested employment (or eligibility to obtain)
• 1+ years of nursing experience
• A hospice heart: compassionate, calm, and dependable in urgent care situations
• Excellent clinical judgment and the ability to provide support in emotionally intense settings
• Comfortable providing care in patients’ homes, facilities, or wherever they reside
• Valid driver’s license, automobile insurance, and reliable transportation
• Current CPR certification

Responsibilities

• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior
• Provide direct patient care and support after normal business hours
• Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift
• Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care
• Support and collaborate with an interdisciplinary group including LPNs/LVNs, CNAs, and other support staff
• Accurately document clinical visits and communications
• Participate in quality assessment and performance improvement programs
• Work independently while ensuring continuity of care and coordination with the daytime care team
• Performs on-going review and follow up on assigned patient clinical records to assure quality services are provided, standards of practice are followed, and compliance with policies, procedures and regulatory requirements is met and maintained
• Practices infection control measures and standard precautions
• Makes routine supervisory visits of, and with, all other team members at least monthly
• Monitors scheduling of all staff
• Maintains a high level of knowledge pertaining to the management of pain and symptoms in the dying process
• Participates in the Quality Assessment and Performance Improvement Program
• May be asked to perform routine visits during normal business hours
• Attends staff meetings
• Performs other activities as assigned

• Completes required Company annual training
