Job summary

Heartland Hospice is seeking an After Hours RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care and peace of mind in the patient’s home or facility.

You will work as the primary liaison between patients, families, physicians, and the hospice team, documenting visits and coordinating with day shift staff to ensure continuity of care and timely management of symptoms.

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• CPR certification and valid driver’s license may be required depending on location.
• Ability to provide compassionate care in urgent situations.
• Familiarity with electronic medical records (EMR) systems is a plus.

Responsibilities

• Provide direct patient care after normal business hours.
• Serve as liaison between patients, families, physicians, and hospice team during shift.
• Respond promptly to urgent calls and manage symptoms per care plan.
• Document visits and communications accurately in EMR.
• Collaborate with interdisciplinary team to ensure continuity of care.
