Job summary

Heartland Hospice is seeking an After Hours RN to respond to patient needs during evenings, nights, weekends and holidays, delivering compassionate care in the home setting where patients reside.

You will act as the primary liaison between patients, families and the hospice team during after-hours shifts, perform assessments, document visits in EMR, manage symptoms, and coordinate care with the daytime staff to ensure uninterrupted quality care.

Qualifications

• Current RN license in the state of requested employment.
• 1+ years of nursing experience.
• Hospice/home health experience is preferred.
• Ability to provide care in patients' homes or facilities.

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays.
• Serve as primary liaison between patients, families, physicians, and the hospice team during your shift.
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care.
• Document clinical visits and communications in EMR.
• Participate in quality assessment and performance improvement programs.
