Job summary

Heartland Hospice is seeking an After Hours RN to provide compassionate hospice care in patients’ homes during evenings, nights, weekends, and holidays. You will collaborate with the interdisciplinary team, document care in EMR, and ensure continuity of care when the day team is unavailable.

You will assess patient needs, educate families, and participate in quality improvement activities while upholding hospice philosophy and policies.

Qualifications

• Current RN license in the state of employment (or eligibility to obtain).
• 1+ years of nursing experience.
• Hospice/home health/oncology/palliative care preferred or exposed.

Responsibilities

• Respond to patient needs after hours and during urgent calls.
• Serve as liaison between patients, families, physicians, and the hospice team.
• Document clinical visits and communications in the EMR.
• Develop and implement the Hospice Plan of Care with the Interdisciplinary Group.
• Educate patients and families on medications, dressings, oxygen, and care needs.
• Coordinate with daytime team to ensure continuity of care.
• Participate in Quality Assessment and Performance Improvement programs.
• Maintain infection control and safety standards.
