Job summary

Heartland Hospice is seeking an After Hours RN to deliver compassionate care in patients' homes during evenings, nights, weekends and holidays. You’ll coordinate with the daytime team and ensure continuity of care.

Responsibilities include assessing patient needs, implementing the Plan of Care, educating families, and documenting thoroughly in EMR while collaborating with the Interdisciplinary Group to support quality outcomes.

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• CPR certification.
• Comfort delivering care in patients' homes or facilities.
• Valid driver’s license and reliable transportation.

Responsibilities

• Report to Administrator or Patient Care Manager Senior.
• Provide direct patient care after hours and on weekends.
• Serve as liaison between patients, families, physicians, and hospice team.
• Respond to urgent calls and manage symptoms per plan of care.
• Document visits and communications in EMR.
• Collaborate with Interdisciplinary Group and participate in QA/PI.
