Job summary

Heartland Hospice is seeking an experienced After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays. You will coordinate with the hospice team, educate families, and document care in the EMR while maintaining high-quality, compassionate care in patients' homes.

Qualifications include an active RN license, 1+ year nursing experience, CPR certification, and the ability to work independently in a home-based setting.

Qualifications

• Current RN license in the state of employment or eligibility to obtain.
• 1+ years of nursing experience.
• A hospice heart: compassionate, calm, and dependable in urgent care situations.
• Excellent clinical judgment and the ability to provide support in emotionally intense settings.
• Comfortable providing care in patients’ homes or wherever they reside.
• Valid driver’s license, automobile insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Respond to patient needs during evenings, nights, weekends, and holidays.
• Serve as the primary liaison between patients, families, physicians, and the hospice team during shifts.
• Document clinical visits and communications accurately in the EMR.
• Evaluate patient and family needs to guide clinical decisions and care plans.
• Collaborate with Interdisciplinary Group to provide comprehensive services.
• Maintain the Plan of Care and education for patients and caregivers.
