Job summary

Heartland Hospice seeks an After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care in patients' homes and facilities.

You'll collaborate with the interdisciplinary team, document visits, and contribute to quality improvement while maintaining the Plan of Care and ensuring continuity of care with daytime staff.

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• Hospice mindset: compassionate, calm, dependable in urgent care.
• Excellent clinical judgment and emotional resilience.
• Comfort providing care in patients’ homes or facilities.
• Valid driver’s license and reliable transportation.
• Current CPR certification.

Responsibilities

• Respond to patient needs after normal business hours in evenings, nights, weekends, and holidays.
• Provide direct patient care and support during shifts.
• Act as primary liaison between patients, families, physicians, and hospice team.
• Document visits and communications accurately.
• Collaborate with interdisciplinary team and participate in QA/PI programs.
• Maintain Plan of Care and educate patients and caregivers as needed.
