Job summary

Heartland Hospice is seeking an After Hours/On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends and holidays in patients’ homes or care facilities. You will provide direct nursing care, coordinate with the interdisciplinary team, and document visits to ensure continuity of care.

The role emphasizes compassionate, autonomous practice, strong clinical judgment, and collaboration with LPNs/LVNs, CNAs, and the daytime care team.

Qualifications

• Must hold current RN license in the state of employment.
• Minimum 1 year of nursing experience is required.
• CPR certification is required.
• Valid driver’s license with reliable transportation is required.

Responsibilities

• Provide direct patient care after normal business hours.
• Serve as the primary liaison between patients, families, physicians and the hospice team.
• Document clinical visits and communications accurately.
• Respond promptly to urgent calls, patient visits and symptom management needs.
• Coordinate with daytime team to ensure continuity of care.
