Heartland Hospice is seeking an After Hours Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care and peace of mind in the home setting. You will work independently, coordinate with daytime teams, document visits, and participate in quality improvement while supporting patients and families during the end-of-life journey.

Current RN license in the state, 1+ year nursing experience, CPR, reliable transportation required;

Formación

• Current RN license in the state of employment or eligibility to obtain.
• 1+ years of nursing experience.
• Hospice heart — compassionate, calm, dependable in urgent care situations.
• Comfortable providing care in patients’ homes or wherever they reside.

Responsabilidades

• Provide direct patient care after normal business hours.
• Serve as primary liaison between patients, families, physicians, and hospice team.
• Respond promptly to urgent calls, patient visits, and symptom management needs.
• Document clinical visits and communications accurately.
• Participate in quality assessment and performance improvement programs.
