Job summary

Heartland Hospice is seeking an After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care in home settings. You will coordinate with the interdisciplinary team, document care, and support families while maintaining the Plan of Care.

Ideal candidates have an active RN license, 1+ year nursing experience, CPR, and reliable transportation.

Qualifications

• Current RN license in the state of employment or eligibility to obtain
• 1+ year nursing experience
• CPR certification current
• Valid driver’s license and reliable transportation
• Compassionate, calm under pressure and able to provide care in patients' homes

Responsibilities

• Provide direct patient care after hours
• Serve as liaison between patients, families, physicians and hospice team
• Respond promptly to urgent calls, visits, and symptom management
• Document visits and communicate with the care team
• Educate patients and families about Plan of Care and medications
• Participate in quality improvement programs
• Coordinate with daytime care team to ensure continuity of care
