Job Purpose

The primary purpose of this role is to provide comprehensive nursing care, including assessment, care planning, clinical emergency management, safe medication administration, infection control, and integrative care. This position also involves supervising nursing aides and healthcare aides in accordance with established policies and procedures.

Job Responsibilities
Patient Care
  • Deliver high‑quality nursing care to residents consistently.
  • Develop, evaluate, and modify individualized care plans for residents.
  • Assess the physical, emotional, and psychological well‑being of residents to ensure optimal care.
  • Conduct regular rounds with staff to monitor residents' health and medical conditions.
  • Supervise and guide staff to ensure adherence to caregiving, nursing, and administrative procedures.
  • Ensure medical assistance aligns with training and Standard Operating Procedures (SOP).
  • Administer first aid and emergency health measures, including CPR.
  • Perform wound and lesion dressing using sterile techniques to minimize infection risks.
  • Monitor and assist with residents' personal care, including hygiene, feeding, positioning, transferring, ambulation, and toileting.
  • Utilize appropriate equipment and lifting devices as needed.
  • Ensure food meets residents' dietary requirements.
  • Be available on‑call for emergencies.
Social Welfare Assistance
  • Collaborate with Social Workers and other professionals to assess and review cases for admission and respite care.
  • Develop and sustain programs that meet residents' needs.
  • Provide counselling and emotional support to residents and their families, responding to inquiries and resolving complaints.
Medical Services
  • ACcompany doctors during rounds, assisting with examinations and providing updates on residents' conditions.
  • Transcribe and execute doctors' orders for medication and treatments.
  • Prepare and administer precise medication dosages as prescribed.
  • Follow up on medical programmes advised by clinics or hospitals.
  • Audit medical records to ensure proper documentation and maintenance.
  • Maintain inventory and ensure proper storage and expiration of medications.
Stakeholder Management
  • Act as a liaison to clients, maintaining good rapport and service quality.
  • Communicate and coordinate with partners (MSF, NCSS, SG Enable) to ensure continuity of service.
  • Maintain positive public relations with volunteers and visitors.
  • Oversee general staff welfare.
Administration
  • Prepare staff duty rosters to ensure adequate coverage.
  • Maintain and prepare documentation, including care plans, healthcare records, and reports.
  • Submit reports to the Head of Nursing and relevant supervisors.
Safe Work Practices
  • Ensure the cleanliness and safety of the Home.
  • Maintain equipment used and conduct quality assurance checks.
  • Familiarise with emergency and evacuation procedures and assist in fire drills.
Other Duties
  • Assist in events held.
  • Promote the Home to optimise resource usage.
  • Encourage volunteer involvement among individuals, groups, and companies.
  • Perform any other duties as assigned by the Reporting Officer.
Job Specifications
Qualifications and Experience
  • Possess a recognised university degree, certificate, or diploma in nursing.
  • Preceptor Certification.
  • Registered Nurse with the Singapore Nursing Board (SNB).
  • 5‑10 years of relevant experience in a hospital or healthcare environment.
  • Experience working with the disabled or elderly.
  • Experience as a preceptor / clinical instructor with training knowledge.
