Job Description
  • Key person in setting up and upkeep of clinical/medical protocols pertaining to client needs, from onboarding to discharging.
  • Assessment for new case referrals and reassessment of existing clients.
  • Guide, train and leading a team of Healthcare Assistants and oversee the nursing care, standards and operations across our day care centres.
  • Working closely with other healthcare professionals on planning client ICP, care and safety in the centre.
  • Respond to emergencies, conduct investigations on incidents and preventive actions.
  • Working with the centre management on ensuring the upkeep of clinical equipment and supplies.
  • Evaluate and monitor progress of clients’ treatment regularly.
  • Ensure strict compliance to MOH regulations in the performance of services.
  • Create and maintain database of all clients, ensuring strict compliance to documentation, providing evidence-based treatment with accurate and timely records.
  • Any other adhoc tasks assigned by the management.
Requirements
  • Minimum NITEC / Diploma / Bachelor’s Degree in Nursing.
  • Advanced Diploma in Gerontology or Palliative Care is an advantage.
  • Must be an Enrolled Nurse or Registered Nurse with Singapore Nursing Board and possess a clean disciplinary record (nurses on conditional licenses are not eligible for this role).
  • Minimum 2 years of clinical experience.
  • Proficiency in Microsoft Office and comfortable using IT tools in daily work.
  • Ability to converse in local dialects will be an added advantage due to elderly clients in eldercare sector.
