Job summary

A recruitment agency specializing in social services is looking for a Social Worker/Caseworker in Singapore. Job Overview

The Social Worker/Caseworker supports residents through care planning, case management, and coordination with internal and external partners to ensure holistic support and well-being.

Key Responsibilities
  • Develop personalised care plans tailored to each resident’s needs.

  • Implement care plans and track residents’ progress regularly.

  • Coordinate support services and interventions with relevant stakeholders, including medical, social service, and community partners.

  • Maintain proper documentation and adhere to organisational SOPs.

  • Provide emotional support, guidance, and referrals where necessary.

  • Participate in multidisciplinary meetings and contribute to care decisions.

Requirements
  • Degree or Graduate Diploma in Social Work, Psychology, Counselling, or related fields.

  • Minimum 1 year of relevant experience in social service, case management, or related functions.

  • Able to follow SOPs, organisational policies, and care procedures.

  • Willing to perform shift duties according to the duty roster, including DO shifts and public holidays.

  • Strong interpersonal, communication, and case management skills.

Work Schedule:

  • Monday to Friday (office hours)

  • Required to perform 1 DO shift per month; Required to work on public holidays
