The Opportunity:

We are looking for Senior / Staff Nurse – Specialty Care (Palliative) (Contract) to join us for various disciplines to provide high quality patient care. The incumbent is expected to deliver patient centric and compassionate care to nursing home residents, their families, and carers. To act as a resource to members of the Acute and Community Health Care Teams, Nursing Homes (NH), Hospice multidisciplinary team and other teams, agencies, and individuals.

Cross site between Ng Teng Fong Hospital (Acute and Community Hospital) and Nursing Homes to enhance smooth transition of care. To take responsibility for the management of a specialist palliative care nursing caseload in the community and to identify, plan, and manage patients’ end of life symptoms in collaboration with the Palliative Care Team.

Job Responsibilities:

The incumbent shall be responsible to:

A) In Acute Setting (work with inpatient palliative team at NTFGH/JCH)

  • Participate in the assessment, development, implementation, evaluation, and documentation of care in accordance with professional guidelines. Take appropriate interventions as required after discussion with palliative team.
  • Develop and display high level of clinical and technical palliative care expertise which encompasses drug management and use of syringe drivers.
  • Collaborate with the MDT to assess patient’s need for physical, psychological, emotional, and spiritual support.
  • Support ward nurses and educate caregivers on dying process and management of patients on end - of- life (including basic nursing procedures to ensure comfort).
  • Review patient care plans and realign goals of care in case of admission to NTFGH.
  • Be the liaison and Point of Contact between the nursing home team case managers and ward team.
  • Coordinate end -of- life care back to the NH or hospice.

B) In Community Settings (Nursing Home)

  • Prioritise and manage a specialist palliative care nursing caseload. Review nursing care plans for NH residents.
  • Participate in regular reviews with nursing home team and palliative team.
  • Attend regular case discussion/ multi-disciplinary/mortality review discussions in NH.
  • Follow up with nursing staff on nursing care plans.
  • Attend to nursing home staff’s queries and provide clarifications.
  • Collaborate with nursing home team to review and implement nursing care pathways and protocols to meet resident’s needs.
  • Liaise between nursing home team and palliative team.
  • Assist in providing training for the nursing home staff.
  • Support nursing home staff in ensuring residents are screened and eligible residents are recruited.

Job Requirements:

  • Diploma in Nursing or State Registered Nurse certificate
  • Relevant Advanced Nursing Diploma or equivalent is preferred for candidate applying for the Senior Staff Nurse position
  • Relevant clinical experience
  • Good interpersonal and communication skills
