Job Description – Senior / Medical Social Worker

Join Our Team at St John's Home for Elderly Persons

Since 1958, St John's Home for Elderly Persons has been a trusted Sheltered Home for seniors under the Ministry of Social and Family Affairs (MSF). As we transition into a Nursing Home under the Ministry of Health (MOH) by 2026, we are seeking a compassionate Medical Social Worker (MSW) to join our team, providing psychosocial care to elderly residents and their families in our Nursing Home and Centre-based services.

Key Responsibilities
  • Reporting to the Director of Nursing/Clinical Governance Officer (CGO).
  • Conduct psychosocial and financial assessments, and provide counselling, emotional support, and psychoeducation to residents and families.
  • Collaborate with the multi-disciplinary team to develop care plans, facilitate family conferences (if required), and help Comms and Partnership Dept to jointly draft replies for any feedback or complaint letters from residents or their Next-of-Kins (NOKs).
  • Develop MSW’s Standing Operating Procedures (SOPs) aligned with the Healthcare Services Act (HCSA).
  • Implement and facilitate programmes to foster support and connection with residents’ NOKs and the community.
  • Plan and initiate funded projects (e.g., sponsorships, government funding), where available.
  • Act as an advocate and liaison, connecting residents and families with internal and external resources, including MOH, AIC, hospitals, and other nursing homes.
  • Liaise with CPF Board for residents' CPF statements and balances.
  • Plan the conduct of Medifund Committee meetings and advise/seek members’ approvals on proposed subsidies for residents.
  • Conduct means testing for potential residents before admission to St John's Home.
  • Identify risks and intervene promptly to ensure residents’ well‑being.
  • Maintain accurate and timely documentation.
  • Participate in quality improvement initiatives and service development.
  • Perform any other tasks assigned by the Director of Nursing/CGO or CEO.
Requirements
  • Master/Bachelor/Post Graduate Diploma in Social Work.
  • Preferably registered with Singapore Association of Social Workers (SASW).
  • 3+ years of experience in healthcare or eldercare.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
