Qualifications

• Registered nurse with at least 3 years of relevant experience.
• Experience in palliative or geriatric care is an advantage.
• Passion for hospice and palliative care.

Responsibilities

• Conduct home visits and perform night call duties when scheduled.
• Prepare and write care plans for new patients.
• Work in collaboration with a multi-disciplinary team.
