Qualifications

Responsibilities

• Review applications and conduct interviews with clients and caregivers.
• Build rapport with clients and educate families on support.
• Provide casework management and coordinate with external agencies.
• Develop and implement individual care plans for clients.
• Execute and monitor progress on care plans.
• Coordinate with PT and OT departments for client comfort.
• Evaluate existing and prospective clients based on needs.
• Conduct casework evaluations and maintain documentation.
• Provide person-centered counseling for clients and families.
• Assess risks of self-harm and manage financial assistance cases.
• Complete any additional tasks assigned by management.
