About the Role
As a Primary Care Coordinator (PCC), you ensure smooth delivery of key services across clinics and mobile sites, working closely with doctors, nurses, and HQ. This role suits candidates passionate about healthcare and providing a positive patient experience, with opportunities for growth.
Key Services You Will Support
- Diabetic Retinal Photography (DRP)
- Diabetic Foot Screening (DFS)
- Nurse Counselling
- Spirometry
Key Responsibilities (Both Levels)
- Coordinate daily service operations, patient registration, and appointment scheduling.
- Provide patient education, guidance, and assist clinicians during clinical services.
- Handle documentation, data entry, filing, and patient enquiries professionally.
- Liaise with clinic staff and HQ on operational and workflow matters.
- Support audits, reports, and other administrative tasks as required.
Additional Responsibilities for Senior PCC
- Ensure adherence to SOPs and service quality standards.
- Handle escalation or workflow issues and propose solutions.
- Support onboarding and guidance for junior PCCs.
- Monitor data accuracy and support higher-level reporting needs.
Requirements
Primary Care Coordinator (PCC) Requirements
- Diploma in any discipline (healthcare/life sciences preferred). Candidates with clinic operations experience will also be considered.
- 1–3 years of healthcare or service-related experience preferred.
- Basic–intermediate Microsoft Office skills.
- Responsible, adaptable, patient-centred, and a strong team player.
Senior Primary Care Coordinator Requirements
- Diploma in Healthcare, Nursing, Life Sciences, Biomedical Science, or related field (or equivalent relevant healthcare operations experience).
- At least 2 years of relevant experience in PCN or healthcare operations preferred.
- Strong organisational & communication skills.
- Ability to manage more complex workflows independently.
- Comfortable with Excel for tracking and reporting (training provided).
- Leadership or mentorship experience is an advantage.

Salary & Benefits
- PCC: $2,500 – $3,600 (depending on experience)
- Senior PCC: $3,600 – $6,000 (depending on experience)
