Job description

Working Hours

• 5 days, 8.30 – 5.30pm (No on call on weekdays)
• One day they will be covering 3-4 homes depends on the patient loads

Jobscope

• This role will be a bit more like case management – manage the care plan for the patients
• You will be the key person who will make the most difference as you make home visits to provide palliative care and recommend psycho-social support to our terminally ill patients and their families.
  You will be part of the most dedicated, compassionate and caring teams comprising doctors, nurses and medical social workers providing an all-in-one professional care to patients at home at all hours, seven days a week.
