Job description

Overview

Monday to Friday, 8.30am to 6pm

Responsibilities

• One day they will be covering 3-5 homes depends on the patient loads
• This role will be abit more like case management – manage the care plan for the patients
• You will be the key person who will make the most difference as you make home visits to provide palliative care and recommend psycho-social support to our terminally ill patients and their families.
• You will be part of the most dedicated, compassionate and caring teams comprising doctors, nurses and medical social workers providing an all-in-one professional care to patients at home at all hours, seven days a week.
