Responsibilities

  • Conduct comprehensive assessments to identify residents’ functional abilities and rehabilitation needs.
  • Develop, implement, and review individualized rehabilitation plans to support mobility, cognitive, and daily living goals.
  • Carry out weekly reviews to evaluate residents’ progress and adjust therapy plans accordingly.
  • Participate in multidisciplinary case discussions to ensure cohesive and coordinated care.
  • Plan and execute discharge plans, ensuring a smooth transition and continuity of care post-discharge.
  • Provide therapy interventions focusing on functional, psychosocial, and adaptive skill development.
  • Recommend and train residents or caregivers in the use of assistive devices and adaptive equipment.
  • Maintain accurate documentation of therapy sessions, assessments, and progress reports.
  • Educate residents, families, and care staff on rehabilitation techniques and strategies for independence.
