A well-established and leading Healthcare company is looking for a dedicated individual to support Healthier SG initiatives within GP clinics and the community. In this role, you will assist with administrative tasks such as coordinating screenings, scheduling appointments, managing patient reminders, and handling deployment reporting.

Job Responsibilities

• Support Healthier SG–related initiatives at GP clinics.
• Liaise with service providers to arrange annual eye and foot screening deployments for patients with diabetes at GP clinics and community nodes.
• Schedule patient appointments for ancillary services, make reminder calls, and assist with payment collection during deployments.
• Partner with GPs on social prescribing by connecting patients to community resources to improve their health and well-being.
• Refer patients to community and social partners for various services, including exercise programmes, support groups, community groups, day care, or home care services.
• Support GPs in coordinating and integrating patient care with community and social agencies.
• Provide basic technical assistance to elderly or less tech‑savvy patients using Healthier SG–related mobile applications (e.g., HealthHub, Healthy 365).

Job Requirements

• Min. Diploma and above
• At least 1 year of patient-fronting experience in a healthcare setting
