We are seeking a proactive and organised Executive to join our Community Care team. In this role, you will support the planning, development, and delivery of patient-centred programmes, ensuring that initiatives are well-designed, effectively implemented, and impactful for the kidney care community. You will also contribute to monitoring programme outcomes, preparing proposals and reports, and fostering collaboration with both internal and external stakeholders. Your responsibilities are as follows:

Programme Planning & Implementation
  • Co-lead the design, rollout, and management of new and existing programmes using a project management framework.
  • Ensure programmes are patient-centred, effective, and aligned with organisational goals.
  • Monitor and evaluate programme outcomes for quality and fidelity.
Stakeholder & Communication
  • Collaborate with internal and external partners to strengthen programme delivery and support.
  • Prepare and present updates, proposals, and evaluation reports for senior management.
  • Draft papers, project documents, purchase requests, invoices, and grant applications.
Operational Support
  • Support team and departmental operations.
  • Assist with change requests, financial updates, and administrative processes.
  • Perform other duties as assigned by the HOD.
Qualifications
  • A degree in any social service related field (Social work, Sociology, Counselling, or Psychology), Finance, Mass Communication and/or New Media are preferred.
Experience

1–2 years of relevant working experience, preferably in the community care sector

Certification or experience in project management is an advantage.
